Who We Support
At United Health Garden LLC, we provide compassionate, person-centered support for individuals with intellectual and developmental disabilities. Our services are designed to promote independence, dignity, personal choice, and meaningful community involvement while ensuring each individual feels safe, respected, and supported.
We support adults receiving Medicaid Waiver services, individuals in need of supervised or supported living, and those who benefit from behavioral, emotional, or daily living support. Our team works closely with families, guardians, case managers, nurses, and interdisciplinary professionals to create individualized care that reflects each person's goals, strengths, and preferences.

Admission Criteria
- Must be eligible for Indiana Medicaid Waiver services
- Must have an approved Person-Centered Individualized Support Plan (PCISP) or service plan
- Must require residential, supported living, or community-based support services
