Who We Serve
United Health Garden proudly serves individuals who need compassionate, person-centered support to live meaningful, safe, and independent lives within their communities. We support individuals with intellectual and developmental disabilities, behavioral health needs, and other care needs by providing quality residential and community-based services tailored to each person's goals, preferences, and strengths.
Admission Criteria
- Must be eligible for Indiana Medicaid Waiver services
- Must have an approved Person-Centered Individualized Support Plan (PCISP) or service plan
- Must require residential, supported living, or community-based support services
